Will administer medications as authorized and scheduled; monitors residents for concerns relative to medication; discusses side effects with physician or director of nursing when necessary to determine if changes should be made. Will submit timely documentation (progress notes, assessments, treatment plans, notes to chart) as outlined in treatment plan. Will educate and communicate with patients on the potential benefits and side effects of medication. Will follow up on health care as prescribed by physician/nurse; will work with patients to develop their awareness and understanding of their symptoms; will make suggestions to help them work through their fears (e.g., shots) by providing information and support. Will complete medical assessments; record history and vital signs; determine information about patient's health. Will provide supervision, protection and care to children admitted to home health care. Will assist health services supervisor with administration and maintenance of patients' immunizations and inputs appropriate data into the California Immunization Registry when needed. Will assist in the administration of tuberculosis screening and is responsible for reading and recording results of screening. Will maintain/control assigned medication supplies; monitor and maintain records in accordance with Agency policies, other requirements, and contracts; coordinates prescriptions, delivery of medications; destruction of medications, maintain/control supply of injection supplies for administering medications. Will ensure complete and accurate documentation of services provided to patient for contract compliance, statistical and financial reporting and other purposes as necessary. guidelines should risk escalate or abuse occur. Assist families in the acquisition of independent living skills; i.e. proper administration of medications, seeking out and utilizing medical community resources. Will attend and participate in department staff, supervision, and support meetings, and in-service programs, consultations and meetings as appropriate.
