Job Title: Health Program Representative
Duration: 3+ Months
Location: Los Angeles, CA

Shift Timings: Monday through Friday 8am -5pm.

Description:
• Friendly, enthusiastic, engaging and ability to plan and facilitate social events.
• Great communication and listening skills. Customer service and healthcare experience. Ability to assess problem solve safety concerns. Candidate that is passionate and compassionate, ability to demonstrate empathy. Medicare and community resources knowledge desired.
• This Person will be physically present in the care center. 80% is working in the togetherness program. Engaging social connections – events that they will facilitate.
• They will be in the care centers to get everyone engaged. Most DR offices you just sit and wait and here there is more a welcoming space and a social space. To help deal with loneliness and isolation.
• Making calls and assessments to people – friendly organic phone call.
• Problem solving – people have different issues and different concerns. Dealing with the customer service issues – will be cross trained in member services so they can help with ID cards, or walk through the process to address issues or connect them to people who can help if not.
• Addressing safety issues and concerns – not a security guard, but just making sure the area is safe because it is a front line area where people come into the clinical. Charismatic, outgoing, (not introverted).
• They will have a computer where they can input and search. Creative side where they can put together materials.
• They will get a lot of training on how to have those conversations.
• It is a bonus if they know about the community resources. Someone who lives in that community.
• Monday-Friday 8-5 is standard. May be times where they might do 9-5:30. Training will be 2 weeks in Cerritos with them and their team.
• Telephonic training as well with another person. If all goes well with what we are doing, this could turn permanent.
• 15-20 miles to the Cerritos office for interviews.

Responsibilities:
• Responsible for providing initial contact between distinct Care Management programs and specific identified members. Under direct supervision handles inbound/outbound enrollment and engagement calls with eligible members to provide information regarding program features.
• Primary duties may include, but are not limited to: Gathers and records appropriate member information in accordance with policies and procedures via telephone. Encourages members to participate in the Care Management programs by providing information about the program, outlining program features/value and explaining available services.
• Records call details and utilize call scripts as needed based on client requirements. Directs member inquiries to appropriate clinical staff. Initiates referrals and manages consults from clinicians for participants registered in Care Management.
• Gathers information from hospitals, health plans, physicians, patients, vendors and other referral sources. Contacts physician offices to obtain demographic information or related data as needed.
• Requires HS diploma or the equivalent; 6 months experience in a telesales or customer service demonstrating ability to influence customers/ members; or any combination of education and experience, which would provide an equivalent background. Computer/keyboarding proficiency and the ability to follow defined policies and protocols required. Knowledge of company products, services and benefits preferred.
