Position Title- Home Attendant
Department Client Support
Reports To Regional Supervisor
Receives feedback from Regional Supervisor, HR Representative and Clinical Team
Employment Status Variable hour/Non-exempt
Qualifications:
Have satisfactorily completed a nursing education program preparing for registered nurse licensure or practical nurse licensure or;
Have satisfactorily completed a nurse aide education program approved by the Virginia Board of Nursing or;
Have certification as a nurse aide issued by the Virginia Board of Nursing or;
Be successfully enrolled in a nursing education program preparing for registered nurse or practical nurse licensure and have currently completed at least one nursing course that includes clinical experience involving direct client care or;
Have satisfactorily passed a competency evaluation program that meets the criteria of 42CFR$484.36(b). Home attendants of personal care services need only be evaluated on the tasks 42 CFR$484.36(b) as those tasks relate to the personal care services to be provided or;
Have satisfactorily completed training the "Personal Care Aide Training Curriculum" dated 2003, of the Department of Medical Assistant Services and provide, at least, two work-related references
Working cell phone with voice mail for ready access
Successful completion of CareFamily Virginia's Alzheimer's and Dementia/Hospice and Advanced Transfer Training
Summary of Job Responsibilities: Responsibilities of the Home Attendant include, but are not limited to, the following:Caregivers must comply with all applicable laws and regulations, and all CareFamily Virginia policies and procedures during the course and scope of their employment with CareFamily Virginia.
Responsibilities and Duties:
The Home Attendant provides only those health related services/tasks within the established job description and the Plan of Care. The duties of the Home Attendant will remain in compliance with state-specific regulations and agency policies and procedures.
Adhering to client's rights
Maintaining client confidentiality and adhering to HIPPA regulations
Providing for and assisting with personal care needs, such as bathing, dressing, grooming, hair care, shaving, oral care, and perineal care
Cleaning and filing fingernails
Assisting with toileting using bedside commode, bedpan, and urinals
Assisting with ambulation including the use of walkers, canes, and wheelchairs as needed
Performing home management functions such as light housekeeping of areas of the residence that the client uses, laundry, bed-making and cleaning
Planning meals, shopping for groceries, preparing and serving food, feeding and clean-up
Responsible for referring to the client's individualized Plan of Care regarding reminding and assisting with client's normally self-administered medications
Reporting changes in the client's condition or family situation to RN Supervisor/Branch staff within 24 hours
Responding to and reporting emergency situations to RN Supervisor/Branch staff
Documenting services provided on the appropriate flow sheet
Accompanying client to schedule appointments
Participating in in-service programs
Submitting documentation within required time frames
Caring for intact skin to prevent skin breakdown
Assisting with transfers to assure client safety
Caring for Alzheimer's patients
Assisting with proper body alignment to prevent complications from immobility
Transporting clients or assisting with errands
Assisting with turning on oxygen concentrator only as directed by the client
Collecting specimens (urine, sputum, and stool)
Monitoring and recording client intake and output
Assisting with ROM or prescribed exercise program
Taking and recording vital signs
Providing ostomy care
Applying Ted hose
Assisting with deep breathing and coughing exercises
Assisting with external urinary drainage
Perform other duties as assigned consistent with the terms of this job description and related to client's independent living where the client cannot perform such duties due to the client's health or age limitations.
Additional Duties after Alzheimer's and Dementia/Hospice and Advanced Transfer Training:
Assisting with Medical lifts
Assist with advanced transfers to assure client safety
Caring for Alzheimer's clients in need of constant redirection
Caring for Dementia clients in need of constant redirection
