In this role, you will conduct initial patient assessments by gathering detailed medical histories, symptoms, and vital information through virtual platforms daily. You will triage cases based on urgency, advising on immediate actions or scheduling provider consultations, handling an average of 30-50 patient interactions per shift. Weekly, collaborate with physicians, nurse practitioners, and care coordinators to review complex cases and refine triage protocols. Monthly responsibilities include analyzing triage data for trends, contributing to quality improvement reports, and participating in team training sessions to maintain high standards. You will document all interactions meticulously in our EHR system, ensure HIPAA compliance, and follow up on patient care plans, working closely with multidisciplinary teams to optimize workflows and patient satisfaction metrics like response time under 5 minutes and resolution rates above 95%.
Candidates must hold an active Registered Nurse (RN) license in good standing, with a BSN preferred and at least 2-3 years of clinical experience in family medicine or primary care. Proficiency in telehealth platforms such as Doxy.me or Zoom for Healthcare, electronic health records like Epic or Cerner, and triage protocols including telephone nursing guidelines from organizations like the American Academy of Ambulatory Care Nursing is essential. Current BLS certification and basic life support knowledge are required, along with familiarity with common family medicine conditions such as hypertension, diabetes, and pediatric illnesses. Experience in high-volume call centers or urgent care settings is highly valued to ensure seamless integration into our remote operations.
Success in this role demands strong clinical assessment skills paired with excellent verbal communication to build rapport with diverse patients over the phone or video. You should exhibit empathy, critical thinking, and the ability to remain calm under pressure during high-acuity situations.
