Medfamily Wellcare Center is seeking a dedicated Nurse Practitioner to join our Visiting Docs team in New York, NY. As a Nurse Practitioner with Visiting Docs, you will conduct comprehensive patient assessments, diagnose acute and chronic conditions, and develop tailored treatment plans during home visits. Daily tasks include performing physical exams, ordering and interpreting diagnostic tests like labs and imaging, prescribing medications, and administering minor procedures such as wound care or vaccinations. Weekly, you will manage a caseload of 15-20 patients, coordinating care transitions with specialists, hospitals, and pharmacies to ensure seamless continuity. Monthly responsibilities encompass reviewing patient progress metrics, participating in team huddles to discuss complex cases, documenting encounters in our EHR system with 100% compliance, and achieving key performance indicators like 95% patient satisfaction scores and timely follow-ups. You will collaborate closely with physicians, nurses, social workers, and administrative staff, occasionally leading training sessions for newer team members on best practices in mobile healthcare delivery. Candidates must hold a Master's or Doctorate degree in Nursing from an accredited program, with current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP (AGPCNP), or similar from ANCC or AANP. A valid New York State Nurse Practitioner license and DEA registration are required, along with at least 2 years of clinical experience in primary care, urgent care, or home health settings. Proficiency in electronic health records (EHR) systems such as Epic or Cerner, telehealth platforms, and mobile diagnostic tools is essential. Strong knowledge of geriatrics, chronic disease management, and preventive care is necessary, with BLS/ACLS certification preferred. Success in this role demands advanced clinical skills in assessment, pharmacology, and evidence-based practice, paired with technical proficiency in remote monitoring devices and health informatics. Essential soft skills include exceptional communication for building patient rapport, empathy in diverse home environments, critical thinking for on-the-spot decisions, and adaptability to varying schedules and locations.
