Job Description

Join the National Association of Latino Healthcare Executives as SUP Ambulatory Social Medicine in Honolulu, HI. Deliver integrated social medicine services in ambulatory settings, addressing social determinants of health to improve patient outcomes for underserved Latino communities.

Full Description

The SUP Ambulatory Social Medicine role at the National Association of Latino Healthcare Executives (NALHE) is pivotal in advancing equitable healthcare delivery. This position focuses on integrating social medicine principles into ambulatory care, directly impacting patient well-being by tackling social determinants of health such as housing, food security, and transportation barriers. Your work will support NALHE's mission to empower Latino leaders in healthcare, fostering healthier communities in Honolulu through innovative, culturally competent programs that bridge clinical care and social support services. In this dynamic role, you will conduct comprehensive patient assessments to identify social needs during ambulatory visits, developing personalized care plans that incorporate community resources and referrals. Daily tasks include collaborating with multidisciplinary teams—physicians, nurses, and case managers—to integrate social interventions into treatment protocols, ensuring seamless coordination. Weekly, you will lead case reviews and track patient progress using metrics like referral completion rates and health outcome improvements. Monthly responsibilities encompass analyzing program data to refine interventions, reporting on key performance indicators such as reduced readmissions and increased service utilization to executive leadership. You will also train staff on social medicine best practices, manage a small team of social workers, and partner with local organizations for resource expansion, delivering measurable impacts on population health. Candidates must hold a Master's degree in Social Work, Public Health, or a related field, with a valid clinical social work license in Hawaii. At least 5 years of experience in ambulatory care settings, particularly with Latino or underserved populations, is required, including proven expertise in social determinants of health interventions. Proficiency in electronic health records (EHR) systems like Epic, data analysis tools such as SPSS, and bilingual skills in Spanish are essential. Knowledge of federal healthcare policies like Medicaid and culturally responsive care models is mandatory, along with any relevant certifications in case management or health equity. Success demands strong technical skills in health informatics and telehealth platforms, paired with exceptional soft skills like empathetic communication, cultural humility, and crisis intervention. You will excel in leadership, fostering team collaboration and advocating for patients across diverse stakeholders. Industry competencies in social medicine frameworks, community health needs assessments, and evidence-based interventions will set you apart, enabling innovative solutions in a fast-paced ambulatory environment. NALHE is a premier organization dedicated to elevating Latino healthcare executives through leadership development, networking, and advocacy. Our Honolulu team operates in a collaborative, mission-driven culture that values diversity, innovation, and work-life balance. With a focus on professional growth in a supportive environment, we offer hybrid work options amid Hawaii's vibrant community, making us unique in promoting inclusive healthcare leadership. We offer competitive compensation, though salary details are not specified at this time, commensurate with experience. Comprehensive benefits include medical, dental, and vision insurance, a 401(k) with employer match, generous PTO starting at 20 days annually, and flexible hybrid scheduling. Additional perks feature professional development stipends, wellness programs, and opportunities for conference attendance, ensuring holistic support for your career and well-being.
