Oak Street Health is seeking a dedicated Medical Doctor specializing in Family Medicine to join our mission-driven team in Chicago, Illinois. This position offers the opportunity to provide comprehensive primary care to underserved communities while working within an innovative healthcare model focused on value-based care and patient outcomes. As a Family Medicine Physician, you will deliver high-quality medical care to patients across the lifespan, managing acute and chronic conditions while emphasizing preventive health and wellness. You will conduct thorough patient evaluations, develop individualized treatment plans, order and interpret diagnostic tests, and prescribe medications as appropriate. Collaboration with multidisciplinary care teams including nurses, social workers, and care coordinators is essential to ensure coordinated, holistic patient care. You will also maintain accurate electronic health records, participate in quality improvement initiatives, and stay current with evidence-based medical practices and guidelines. The ideal candidate must possess a Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution, along with board certification or eligibility in Family Medicine. An active, unrestricted medical license in the state of Illinois is required, as well as current DEA registration. Candidates should have excellent clinical judgment, strong communication and interpersonal skills, and a genuine commitment to serving diverse and vulnerable patient populations. Previous experience in outpatient primary care, community health, or value-based care settings is highly preferred. Key skills for success in this role include clinical expertise in family medicine, proficiency with electronic health record systems, cultural competency, and the ability to work collaboratively within an interdisciplinary team environment.
