Job Overview
Theoria Medical is seeking a dedicated PRN Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Colby, KS. This per diem role offers flexibility for experienced advanced practice providers who thrive in post-acute and long-term care settings. As a PRN provider, you will deliver compassionate, patient-centered care to residents in skilled nursing facilities, focusing on preventive health, acute illness management, and chronic disease optimization. Key Responsibilities
Conduct comprehensive patient assessments, including history taking, physical exams, and diagnostic evaluations in long-term care environments. Develop and implement individualized treatment plans, prescribe medications, order and interpret lab tests, imaging, and other diagnostics. Manage acute and chronic conditions such as diabetes, hypertension, infections, and wounds, while emphasizing preventive care and health promotion. Collaborate with facility staff, including nurses, administrators, and specialists, to coordinate care and ensure seamless transitions. Document encounters accurately in electronic health records (EHR), participate in care plan meetings, and adhere to evidence-based guidelines. Respond promptly to on-call needs, perform rounds, and provide consultations as required. Maintain compliance with state and federal regulations, including those from CMS and DEA.
Required Qualifications
Active, unrestricted Nurse Practitioner or Physician Assistant license in Kansas. Minimum of 1-2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings. Board certification (ANCC, AANP for NPs; NCCPA for PAs). Valid DEA registration and ability to prescribe controlled substances. Current BLS certification; ACLS preferred. Reliable transportation and willingness to travel within Colby and surrounding areas.
