Job Overview
Theoria Medical is seeking dedicated PRN Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our dynamic team in Wilson. As a PRN provider, you will deliver comprehensive primary care services on an as-needed basis to patients in long-term care and skilled nursing facilities. This role offers the flexibility of per diem work while allowing you to make a meaningful impact on underserved populations, focusing on holistic care, chronic disease management, and preventive health strategies in a collaborative environment.
Key Responsibilities
Conduct thorough patient assessments, including history taking, physical exams, and diagnostic evaluations to develop individualized care plans. Prescribe medications, order and interpret lab tests, imaging, and other diagnostics, while coordinating with interdisciplinary teams for seamless patient care. Manage acute and chronic conditions such as diabetes, hypertension, COPD, and infections, emphasizing evidence-based practices and patient education. Perform procedures like wound care, joint injections, and minor surgeries as needed. Document all encounters accurately in electronic health records (EHR), ensuring compliance with regulatory standards. Collaborate with facility staff, physicians, and specialists to optimize resident health and transition care. Participate in on-call rotations and quality improvement initiatives to elevate facility-wide care standards.
Required Qualifications
Active, unrestricted Nurse Practitioner or Physician Assistant licensure in the state of operation. Master's degree or higher from an accredited NP or PA program. National certification (e.g., ANCC, AANP for NPs; NCCPA for PAs). Minimum of 2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings. Current DEA registration and BLS/ACLS certification. Valid driver's license and reliable transportation for facility visits.
