Matrix Medical Network is seeking dedicated healthcare professionals to join our clinical team as PRN Nurse Practitioners or Physician Assistants specializing in in-home health assessments. This position offers the opportunity to provide high-quality, patient-centered care directly in the comfort of patients' homes throughout the Stratford, Iowa area. Our clinicians play a vital role in bridging gaps in healthcare access by delivering preventive screenings, chronic disease management evaluations, and comprehensive health assessments that support better patient outcomes.
As a PRN clinician, you will conduct thorough in-home visits that include detailed health histories, physical examinations, medication reviews, and health risk assessments.
