Job Overview
Theoria Medical is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Colby, KS. In this full-time position, you will deliver exceptional primary and urgent care services to patients in post-acute and long-term care facilities. Key Responsibilities
Conduct thorough patient assessments, including history taking, physical examinations, and diagnostic evaluations. Develop and implement individualized treatment plans, prescribe medications, order and interpret lab tests and imaging, and perform minor procedures. Manage chronic conditions such as diabetes, hypertension, and heart disease, while addressing acute illnesses and preventive care needs. Coordinate care transitions, participate in care plan meetings, and provide education to patients, families, and staff on disease management and wellness. Maintain accurate electronic health records (EHR), ensure compliance with regulatory standards, and contribute to quality improvement initiatives. Respond to on-call needs as required to support facility patients.
Required Qualifications
Active, unrestricted Nurse Practitioner or Physician Assistant license in Kansas. Master's degree from an accredited NP or PA program. National certification (e.g., ANCC, AANP for NPs; NCCPA for PAs). Minimum of 1-2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings. Current DEA registration and BLS certification; ACLS preferred. Valid driver's license and reliable transportation for travel between facilities in the Colby area.
