Key Responsibilities Conduct comprehensive patient assessments, diagnose acute and chronic conditions, and develop individualized treatment plans in home visits across the Denver metro area. Prescribe medications, order and interpret diagnostic tests, and collaborate with physicians, nurses, and specialists to ensure coordinated care. Educate patients and families on disease management, preventive health measures, and lifestyle modifications to promote wellness. Document patient encounters accurately in electronic health records (EHR), maintain compliance with HIPAA and state regulations, and participate in quality improvement initiatives. Respond to urgent care needs, perform minor procedures, and contribute to on-call rotations as needed to support 24/7 patient access. Required Qualifications Active licensure as a Nurse Practitioner or Physician Assistant in Colorado, with national certification (ANCC, AANP, or NCCPA). Minimum of 2 years of primary care experience, preferably in geriatrics, home health, or adult medicine. Master's degree from an accredited NP or PA program. Valid DEA registration and ability to prescribe controlled substances. Current BLS certification; ACLS preferred. Skills Excellent clinical judgment and diagnostic skills, with proficiency in managing complex comorbidities. Strong interpersonal and communication abilities to build rapport with diverse patient populations. Tech-savvy with experience in EHR systems like Epic or Cerner, telehealth platforms, and mobile documentation tools. Cultural competence and empathy for serving elderly and homebound patients. Organizational skills to manage a flexible schedule of field visits and administrative tasks.
