Job Overview Theoria Medical is seeking a dedicated PRN Nurse Practitioner (NP) or Physician Assistant (PA) to provide exceptional medical care in Cottonwood Falls, Kansas. This per diem role offers flexibility for advanced practice providers passionate about improving patient outcomes in post-acute and long-term care settings. As a key member of our interdisciplinary team, you will deliver comprehensive primary care, manage chronic conditions, and address acute health issues, all while upholding the highest standards of clinical excellence and patient-centered care. Key Responsibilities Conduct thorough patient assessments, including history taking, physical examinations, and diagnostic evaluations to develop accurate diagnoses and personalized treatment plans. Prescribe medications, order and interpret laboratory tests, imaging studies, and other diagnostics, collaborating closely with physicians, nurses, and facility staff to optimize care delivery. Manage a diverse patient population, focusing on geriatrics with common conditions such as diabetes, hypertension, dementia, and respiratory illnesses, while responding effectively to urgent medical needs. Document all encounters meticulously in electronic health records (EHR), ensuring compliance with regulatory standards, billing requirements, and quality metrics. Educate patients and families on disease management, preventive care, and lifestyle modifications to promote long-term health and reduce readmissions. Participate in care conferences, quality improvement initiatives, and on-call rotations as needed to support facility-wide health goals. Required Qualifications Active, unrestricted Nurse Practitioner or Physician Assistant license in Kansas, with national certification (e.g., ANCC, AANP, NCCPA). Minimum of 1-2 years of clinical experience, preferably in primary care, geriatrics, or long-term care settings. Current DEA registration and BLS/ACLS certification. Proficiency in EHR systems and telehealth platforms. Valid driver's license and reliable transportation for travel to assigned facilities in the Cottonwood Falls area.
