Key Responsibilities Provide comprehensive primary care services to patients in skilled nursing and long-term care facilities, including conducting physical exams, diagnosing illnesses, prescribing medications, and developing treatment plans. Perform routine administrative tasks such as managing patient records, coordinating with interdisciplinary teams, ensuring compliance with regulatory standards, and participating in quality improvement initiatives. Collaborate with physicians, nurses, and facility staff to optimize patient care delivery, conduct on-site rounds, order and interpret diagnostic tests, and educate patients and families on health management. Handle documentation in electronic health records (EHR) systems, assist in care transitions, and support telemedicine initiatives as needed to enhance accessibility. Required Qualifications Active, unrestricted Nurse Practitioner or Physician Assistant license in Kansas; Master's degree from an accredited NP or PA program; National certification (e.g., ANCC, AANP for NPs; NCCPA for PAs); Minimum of 2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings; DEA registration and BLS certification required. Valid driver's license and ability to travel between facilities in the Chase area.
