Job Overview Salud Family Health is seeking a dedicated Nurse Practitioner or Physician Assistant to join our healthcare team in Fort Lupton, Colorado. This full-time position offers an excellent opportunity to provide comprehensive primary care services to a diverse patient population in a community health center environment. The successful candidate will work collaboratively with physicians, nurses, and other healthcare professionals to deliver high-quality, patient-centered care that addresses both acute and chronic health conditions. Key Responsibilities Provide comprehensive primary care services including physical examinations, diagnostic evaluations, and treatment planning for patients across the lifespan. Manage acute and chronic illnesses through evidence-based medical interventions and appropriate referrals. Develop and implement individualized care plans that promote health maintenance and disease prevention. Order and interpret diagnostic tests, prescribe medications within scope of practice, and educate patients on health management strategies. Maintain accurate and timely documentation in electronic health records while ensuring compliance with regulatory requirements. Collaborate with multidisciplinary teams to coordinate patient care and participate in quality improvement initiatives. Engage in community outreach activities to promote health education and wellness programs. Required Qualifications Current and unrestricted license as a Nurse Practitioner or Physician Assistant in the state of Colorado. National certification from an accredited certifying body appropriate to the role. Minimum of two years clinical experience in primary care, family medicine, or internal medicine preferred. Current BLS and ACLS certifications required. Valid DEA registration and prescriptive authority in Colorado. Skills Strong clinical assessment and diagnostic abilities with excellent decision-making skills under pressure. Proficiency in electronic health record systems and medical documentation standards. Exceptional communication and interpersonal skills for effective patient interaction and team collaboration. Cultural competency and sensitivity to diverse patient populations.
