Matrix Medical Network is currently seeking a dedicated Nurse Practitioner or Physician Assistant to join our team on a PRN basis in Lancaster, Kansas. This role is designed for experienced healthcare professionals who are passionate about delivering high-quality, patient-centered care in community and home settings. As a vital member of our clinical team, you will conduct comprehensive health assessments, identify chronic conditions, and develop personalized care plans that improve patient outcomes and support better health management across diverse populations. Key Responsibilities include performing thorough in-home and community-based health evaluations, reviewing patient medical histories, and conducting physical examinations to identify potential health risks. You will collaborate closely with physicians, care coordinators, and other healthcare providers to ensure seamless care transitions and appropriate follow-up. Additional duties involve educating patients and their families about chronic disease management, preventive care strategies, and available healthcare resources while maintaining accurate and timely clinical documentation in electronic health records. Required Qualifications for this position include an active Nurse Practitioner or Physician Assistant license in the state of Kansas, along with national board certification and a minimum of one year of clinical experience in primary care, geriatrics, or related specialties. Candidates must possess a valid driver’s license and reliable transportation to conduct visits across various locations in the Lancaster area. Essential Skills for success in this role include strong clinical assessment abilities, excellent communication and interpersonal skills, proficiency with electronic medical record systems, and the capacity to work independently while managing a flexible schedule. Familiarity with Medicare guidelines and value-based care models is highly preferred.
