Job Overview Ascension, a leading non-profit Catholic health system, is seeking a dedicated Registered Nurse (RN) for the Transfer Center at our Elk Grove Village, IL location. In this critical role, you will serve as the central hub for coordinating patient transfers between facilities, ensuring timely and safe patient movement while upholding the highest standards of clinical care. Key Responsibilities You will triage incoming transfer requests, conducting rapid clinical assessments via phone or electronic systems to determine appropriate bed assignments and transport needs. Responsibilities include real-time communication with sending and receiving facilities, documenting transfer details accurately in electronic health records, and monitoring bed availability across the Ascension network. You will escalate complex cases to physicians, ensure regulatory compliance with EMTALA and other standards, and participate in quality improvement initiatives to enhance transfer processes. Additionally, you will provide clinical consultation to support decision-making and contribute to multidisciplinary team huddles during high-volume periods. Required Qualifications Candidates must hold an active Registered Nurse license in Illinois, with a BSN preferred and a minimum of 2 years of acute care nursing experience, ideally in critical care, emergency, or telemetry settings. Current ACLS and BLS certifications are required, along with proficiency in electronic medical records (EMR) systems such as Epic. Excellent communication skills and the ability to thrive in a 24/7 shift environment are essential.
