As a Patient Resource Specialist - LPN, you will play a pivotal role in our remote team, delivering compassionate support to patients by coordinating care resources, managing medications, and facilitating seamless communication between patients, families, and providers. Key Responsibilities
In this role, you will triage patient inquiries via phone, chat, and video, providing clinical guidance within LPN scope of practice. You will review and reconcile medications, educate patients on treatment plans, and connect them to community resources for social determinants of health like transportation and housing. Additionally, you will document interactions in our electronic health record system, collaborate with physicians and care teams on care plans, and monitor patient adherence to ensure optimal outcomes. Expect to handle high-volume caseloads with empathy, accuracy, and efficiency in a fast-paced virtual environment.
Required Qualifications
Active, unrestricted LPN license in good standing is required, with at least 2 years of clinical experience in geriatrics, primary care, or home health. Proficiency in electronic medical records (EMR) systems and telehealth platforms is essential. You must hold a high school diploma or GED, with completion of an accredited LPN program. BLS certification and basic computer literacy, including Microsoft Office and video conferencing tools, are mandatory.
