Join the dedicated healthcare team at Salud Family Health Centers as a Nurse Practitioner or Physician Assistant in Sterling, Colorado. This full-time clinical role offers the opportunity to deliver comprehensive primary care services to patients across the lifespan in a community health setting. You will assess, diagnose, and treat acute and chronic conditions while promoting preventive health measures and patient education. Working collaboratively with physicians, nurses, and support staff, you will contribute to improving health outcomes for diverse and underserved populations throughout northeastern Colorado. Key responsibilities include conducting thorough patient histories and physical examinations, ordering and interpreting diagnostic tests, prescribing medications, and developing individualized treatment plans. You will manage chronic diseases such as diabetes, hypertension, and asthma, provide urgent care for minor illnesses and injuries, and perform routine wellness visits including immunizations and health screenings. Additional duties involve documenting care in the electronic health record, participating in quality improvement initiatives, and supporting care coordination with specialists and community resources. Required qualifications include current licensure as a Nurse Practitioner or Physician Assistant in Colorado, national certification, and at least one year of clinical experience preferably in primary care or family medicine. Candidates must hold a valid DEA registration and demonstrate strong clinical judgment, excellent communication skills, and cultural competence. Bilingual proficiency in English and Spanish is highly valued but not required. Essential skills for success in this role include advanced clinical assessment, diagnostic reasoning, patient education, and collaborative teamwork within a multidisciplinary environment. Proficiency with electronic health records and a commitment to evidence-based practice are essential.
