Key responsibilities include conducting thorough patient histories and physical examinations, ordering and interpreting diagnostic tests, prescribing medications, and developing individualized treatment plans. You will manage chronic diseases such as diabetes, hypertension, and asthma, provide urgent care for minor illnesses and injuries, and perform routine wellness visits including immunizations and health screenings. Additional duties involve documenting care in the electronic health record, participating in quality improvement initiatives, and supporting care coordination with specialists and community resources. Required qualifications include current licensure as a Nurse Practitioner or Physician Assistant in Colorado, national certification, and at least one year of clinical experience preferably in primary care or family medicine.
