Job Overview Salud Family Health, a leading community health center in Sterling, Colorado, is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our multidisciplinary team. This full-time position offers a $10K sign-on bonus and focuses on delivering comprehensive primary care services to underserved populations, including families, adults, and pediatric patients. As a key provider in our Federally Qualified Health Center (FQHC), you will play a vital role in improving health outcomes through patient-centered care, preventive services, and chronic disease management. Key Responsibilities Conduct thorough patient assessments, diagnose acute and chronic conditions, and develop individualized treatment plans. Prescribe medications, order and interpret diagnostic tests, and perform minor procedures such as suturing, biopsies, and joint injections. Collaborate with physicians, nurses, behavioral health specialists, and support staff to coordinate holistic care. Educate patients on health maintenance, disease prevention, and lifestyle modifications. Maintain accurate electronic health records (EHR) in compliance with HIPAA and FQHC standards. Participate in quality improvement initiatives, community outreach, and on-call rotations as needed to ensure 24/7 access to care. Required Qualifications Master's or Doctorate degree in Nursing (for NPs) or Physician Assistant Studies from an accredited program. Current Colorado NP or PA licensure, DEA registration, and national certification (ANCC, AANP, or NCCPA). Minimum of 2 years of clinical experience in primary care, family medicine, or internal medicine preferred. BLS certification required; ACLS preferred. Spanish bilingual skills are highly desirable to serve our diverse patient base. Skills Strong clinical judgment and decision-making abilities. Excellent communication and interpersonal skills for building rapport with patients from varied backgrounds. Proficiency in EHR systems like Epic or eClinicalWorks.
