Matrix Medical Network is seeking dedicated Nurse Practitioners and Physician Assistants to join our clinical team on a PRN basis. In this role, you will conduct thorough in-home health assessments for patients in Stratford, Texas, supporting better health outcomes through preventive care and early detection. Our assessments are designed to identify chronic conditions and risk factors, empowering patients and their primary care providers with valuable health insights. Key responsibilities include performing comprehensive physical examinations, reviewing medical histories, conducting health screenings, and documenting clinical findings accurately in electronic health records. You will also educate patients about their health conditions, promote wellness strategies, and coordinate follow-up care with other healthcare providers as needed. Travel within the assigned geographic area is required to deliver services directly in patients' homes. Required qualifications include an active Nurse Practitioner or Physician Assistant license in the state of Texas, national board certification, and a minimum of one year of clinical experience in primary care, internal medicine, or geriatrics. Candidates must possess a valid driver's license, reliable transportation, and the ability to work independently in diverse home environments. Essential skills for success include strong clinical assessment abilities, excellent communication and interpersonal skills, proficiency with electronic medical records, attention to detail, and a patient-centered approach to care. Experience with geriatric populations and chronic disease management is highly valued.
