This full-time clinical role offers the opportunity to work in a collaborative, patient-centered environment where your expertise will directly impact the health and well-being of individuals and families who need it most. You will assess, diagnose, and treat a wide range of acute and chronic conditions while building lasting relationships with patients across the lifespan. Your key responsibilities will include conducting comprehensive patient evaluations, ordering and interpreting diagnostic tests, prescribing medications, and developing individualized treatment plans in accordance with evidence-based guidelines. You will also provide preventive care services such as health screenings, immunizations, and patient education on lifestyle modifications and disease management. Collaboration with physicians, nurses, behavioral health specialists, and other healthcare professionals is essential to ensure coordinated, holistic care. Additionally, you will maintain accurate electronic health records, participate in quality improvement initiatives, and contribute to community outreach programs that expand access to care for underserved populations. To be successful in this role, candidates must hold current licensure as a Nurse Practitioner or Physician Assistant in the state of Colorado, along with national certification from an accredited body. A minimum of two years of clinical experience in primary care, family medicine, or internal medicine is strongly preferred. Strong clinical assessment skills, proficiency with electronic health record systems, and the ability to work effectively in a fast-paced, team-oriented environment are essential. Bilingual skills in English and Spanish are highly valued but not required.
