Job Overview Matrix Medical Network is looking for a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team on a PRN (as-needed) basis in Wakefield, NE. In this role, you will provide comprehensive primary healthcare services to patients in various community settings, including homes, senior living facilities, and wellness centers. As a key member of our mobile healthcare team, you will conduct thorough assessments, diagnose conditions, prescribe treatments, and promote preventive care to improve patient outcomes and quality of life. Key Responsibilities Your primary duties will include performing detailed health assessments, ordering and interpreting diagnostic tests such as labs and imaging, developing and implementing individualized care plans, and prescribing medications and therapies as appropriate. You will educate patients and families on health management, chronic disease control, and lifestyle modifications to foster long-term wellness. Collaborating with interdisciplinary teams, including physicians, nurses, and social workers, you will ensure coordinated care delivery. Additionally, you will maintain accurate electronic health records, adhere to clinical protocols, and participate in quality improvement initiatives to uphold the highest standards of care. Required Qualifications Candidates must hold a current, active Nurse Practitioner or Physician Assistant license in Nebraska, with national certification from an accredited body such as ANCC, AANP, or NCCPA. A Master's degree or higher in Nursing or Physician Assistant Studies is required, along with at least 2 years of clinical experience in primary care, geriatrics, or adult medicine. Valid DEA registration and BLS certification are mandatory. Proficiency with electronic medical records (EMR) systems and mobile health technologies is essential.
