Job Overview: Matrix Medical Network is looking for a skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our team on a PRN basis in Bridgewater, South Dakota. In this role, you will provide expert medical care to diverse patient populations, primarily in home, long-term care, or community health settings. Key Responsibilities: Perform comprehensive health assessments, including physical exams, medical history reviews, and diagnostic evaluations for adult and geriatric patients. Diagnose and treat acute and chronic conditions, prescribe medications, and order or interpret lab tests, imaging, and other diagnostics. Develop and implement individualized care plans, coordinating with primary care providers, specialists, and multidisciplinary teams to ensure continuity of care. Educate patients and families on disease management, preventive health measures, and lifestyle modifications. Document all encounters accurately in electronic health records (EHR) systems, adhering to clinical guidelines and regulatory standards. Maintain compliance with state and federal regulations, including HIPAA and OSHA standards.
Required Qualifications: Active, unrestricted Nurse Practitioner or Physician Assistant license in South Dakota. Master's degree or higher from an accredited NP or PA program. National certification as an NP (e.g., ANCC, AANP) or PA (NCCPA). Minimum of 2 years of clinical experience, preferably in primary care, geriatrics, or home health. Current DEA registration and BLS certification; ACLS preferred. Valid driver's license and reliable transportation for field-based visits. Ability to pass background checks and meet credentialing requirements.
