Job Overview Theoria Medical is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our innovative healthcare team in Eskridge, Kansas. In this pivotal role, you will provide comprehensive primary and urgent care services to patients in long-term care facilities and skilled nursing communities. This position offers the flexibility of a field-based role with a focus on delivering personalized, evidence-based medicine in a supportive environment. Key Responsibilities Conduct thorough patient assessments, including history taking, physical examinations, and diagnostic evaluations to develop accurate treatment plans. Prescribe medications, order and interpret diagnostic tests such as labs, imaging, and EKGs, and perform minor procedures like wound care, joint injections, and suturing. Manage chronic conditions such as diabetes, hypertension, and COPD, while addressing acute illnesses and preventive care needs. Collaborate with facility staff, physicians, and specialists to coordinate care transitions, develop care plans, and ensure compliance with regulatory standards. Document all patient encounters meticulously in our electronic health record (EHR) system, participate in quality improvement initiatives, and educate patients and families on health management. Respond to on-call needs as required to provide timely interventions and maintain continuity of care. Required Qualifications Active, unrestricted Nurse Practitioner or Physician Assistant license in Kansas; Board certification (ANCC, AANP, NCCPA) required. Minimum of 1-2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings. DEA registration and current BLS certification; ability to obtain facility-specific credentials. Valid driver's license and reliable transportation for travel between facilities in the Eskridge area. Skills Exceptional clinical judgment and diagnostic skills with proficiency in managing complex, multimorbid patients. Strong communication and interpersonal abilities to build rapport with patients, families, and healthcare teams. Tech-savvy with experience in EHR systems like PointClickCare or Epic; detail-oriented with excellent organizational skills. Ability to thrive in a fast-paced, autonomous environment while demonstrating empathy, cultural competence, and a commitment to patient-centered care. Problem-solving prowess and adaptability to evolving healthcare protocols.
