Job Overview
Theoria Medical is seeking a dedicated Full-Time Nurse Practitioner (NP) or Physician Assistant (PA) to join our dynamic team in Columbia, Missouri. In this pivotal role, you will provide comprehensive primary care services to patients in post-acute and long-term care settings, such as skilled nursing facilities and assisted living communities. Key Responsibilities
You will conduct thorough patient assessments, including history taking, physical examinations, and diagnostic evaluations to develop and implement individualized care plans. Prescribe medications, order and interpret diagnostic tests such as labs, imaging, and EKGs, and perform minor procedures while adhering to evidence-based guidelines. Collaborate closely with facility staff to manage transitions of care, coordinate specialist referrals, and address acute illnesses or exacerbations of chronic conditions. Maintain accurate electronic health records (EHR) documentation, ensure compliance with state and federal regulations, and participate in quality improvement initiatives. Additionally, you will educate patients and families on health management, preventive strategies, and lifestyle modifications to promote wellness.
Required Qualifications
Candidates must hold a valid Missouri Nurse Practitioner or Physician Assistant license, with national certification from ANCC, AANP, NCCPA, or equivalent. A Master's or Doctorate degree in Nursing or Physician Assistant studies from an accredited program is required, along with at least 1-2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings. Current DEA registration and BLS certification are mandatory, with ACLS preferred. Strong knowledge of geriatric medicine, common chronic conditions like diabetes, hypertension, and dementia, and familiarity with Medicare/Medicaid guidelines is essential.

Exceptional clinical judgment and decision-making abilities are crucial, complemented by excellent communication and interpersonal skills to build rapport with diverse patient populations and multidisciplinary teams. Proficiency in EHR systems, such as Epic or Cerner, and telehealth platforms is necessary.
