Maintain accurate electronic health records (EHR) documentation, ensure compliance with state and federal regulations, and participate in quality improvement initiatives. Additionally, you will educate patients and families on health management, preventive strategies, and lifestyle modifications to promote wellness. Required Qualifications Candidates must hold a valid Missouri Nurse Practitioner or Physician Assistant license, with national certification from ANCC, AANP, NCCPA, or equivalent. A Master's or Doctorate degree in Nursing or Physician Assistant studies from an accredited program is required, along with at least 1-2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings. Current DEA registration and BLS certification are mandatory, with ACLS preferred. Strong knowledge of geriatric medicine, common chronic conditions like diabetes, hypertension, and dementia, and familiarity with Medicare/Medicaid guidelines is essential. Skills Exceptional clinical judgment and decision-making abilities are crucial, complemented by excellent communication and interpersonal skills to build rapport with diverse patient populations and multidisciplinary teams.
