Job Overview
Theoria Medical is seeking dedicated Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our team in Eskridge, Kansas. In this pivotal role, you will provide high-quality, patient-centered primary care services primarily in long-term care facilities, skilled nursing homes, and assisted living communities. Key Responsibilities
Conduct comprehensive patient assessments, including history taking, physical examinations, and diagnostic evaluations to develop accurate diagnoses. Order, perform, and interpret diagnostic tests such as laboratory work, imaging, and EKGs, prescribing appropriate medications and therapies in compliance with state regulations. Manage chronic diseases like diabetes, hypertension, and COPD through evidence-based treatment plans, patient education, and coordinated care transitions. Perform minor procedures, wound care, and vaccinations, while documenting all encounters thoroughly in electronic health records (EHR) systems. Collaborate with physicians, nurses, facility staff, and families to optimize care plans, participate in quality improvement initiatives, and ensure adherence to infection control and safety protocols. Provide on-call support as needed and contribute to Theoria Medical's telehealth services for remote consultations.
Required Qualifications
Active, unrestricted Nurse Practitioner (ARNP or MSN) or Physician Assistant (PA-C) license in Kansas, with national certification from ANCC, AANP, or NCCPA. Minimum of 2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings. Valid DEA registration and current BLS/ACLS certification. Proficiency with EHR platforms like PointClickCare or Epic, and familiarity with Medicare/Medicaid guidelines. Strong commitment to patient advocacy and cultural competency in diverse populations.
