Full Description
Salud Family Health is seeking a dedicated Nurse Practitioner or Physician Assistant to join our healthcare team in Sterling, Colorado. This part-time position offers the opportunity to provide comprehensive primary care services to patients across the lifespan while working in a supportive, mission-driven environment focused on improving community health outcomes.
Key Responsibilities include conducting thorough patient assessments, diagnosing and treating acute and chronic conditions, prescribing medications and developing treatment plans, performing routine health screenings and preventive care services, maintaining accurate and timely electronic health records, collaborating with physicians and healthcare team members to ensure coordinated patient care, educating patients on health management and disease prevention, and participating in quality improvement initiatives.
Required Qualifications include current Nurse Practitioner or Physician Assistant licensure in Colorado, board certification in the appropriate specialty, minimum of two years clinical experience preferably in primary care or family medicine, current DEA registration, BLS certification, and excellent communication and interpersonal skills.
Skills required for success in this role include strong clinical assessment abilities, proficiency with electronic health record systems, cultural competency in serving diverse patient populations, time management and organizational skills, and the ability to work independently while collaborating effectively with multidisciplinary teams.
