Job Overview
Theoria Medical is seeking a dedicated Full-Time Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Eskridge, KS. In this rewarding role, you will provide comprehensive primary care services to patients in long-term care and post-acute facilities, focusing on preventive care, chronic disease management, and acute illness treatment. As a key member of our interdisciplinary team, you will collaborate with physicians, nurses, and facility staff to enhance patient quality of life and outcomes in a supportive, patient-centered environment.
Key Responsibilities
Conduct thorough patient assessments, including history taking, physical examinations, and diagnostic evaluations. Develop and implement individualized treatment plans, prescribing medications, ordering lab tests, imaging studies, and other diagnostics as needed. Manage chronic conditions such as diabetes, hypertension, and COPD, while addressing acute issues like infections, injuries, and exacerbations. Perform procedures including wound care, joint injections, and minor surgeries when appropriate. Coordinate care with specialists, ensure compliance with facility protocols, and participate in care plan meetings. Document all encounters accurately in electronic health records (EHR) systems, maintaining HIPAA compliance. Educate patients and families on health management, preventive care, and lifestyle modifications. Respond to on-call needs as required to support facility operations.
Required Qualifications
Active, unrestricted Nurse Practitioner or Physician Assistant license in Kansas. National certification (ANCC, AANP for NPs; NCCPA for PAs). Minimum of 2 years of clinical experience, preferably in primary care, geriatrics, or long-term care settings. DEA registration and current BLS/ACLS certification. Valid driver's license and reliable transportation for travel between facilities in the Eskridge area.
