Manage chronic conditions such as diabetes, hypertension, and heart disease through ongoing monitoring, medication adjustments, and lifestyle counseling. Address acute health issues, perform minor procedures, and coordinate referrals to specialists when necessary. Collaborate with facility staff to optimize care transitions, participate in care plan meetings, and educate patients and families on health management. Document all encounters accurately in electronic health records (EHR) systems, maintaining compliance with regulatory standards like HIPAA and state guidelines. Stay current with medical advancements through continuous education and contribute to quality improvement initiatives within the practice. Required Qualifications Active, unrestricted Nurse Practitioner or Physician Assistant license in Missouri. Master's degree or higher from an accredited NP or PA program. National certification as an NP (e.g., ANCC, AANP) or PA (NCCPA). Minimum of 1-2 years of clinical experience, preferably in primary care, geriatrics, or long-term care settings. Valid DEA registration and current BLS certification; ACLS preferred. Proficiency with EHR systems and telehealth platforms. Ability to travel within Windsor and surrounding areas as needed.
