Job Overview Theoria Medical is seeking a dedicated Part-Time Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Windsor, MO. In this role, you will provide comprehensive primary care services to patients in long-term care facilities, focusing on chronic disease management, preventive care, and acute illness treatment. As a key member of our interdisciplinary team, you will collaborate with physicians, nurses, and facility staff to enhance patient outcomes in a supportive, patient-centered environment. Key Responsibilities Conduct thorough patient assessments, including history taking, physical examinations, and diagnostic evaluations to develop individualized care plans. Prescribe medications, order and interpret lab tests, imaging studies, and other diagnostics, ensuring evidence-based treatment aligned with best practices. Manage chronic conditions such as diabetes, hypertension, and heart disease through ongoing monitoring, medication adjustments, and lifestyle counseling. Address acute health issues, perform minor procedures, and coordinate referrals to specialists when necessary. Collaborate with facility staff to optimize care transitions, participate in care plan meetings, and educate patients and families on health management. Document all encounters accurately in electronic health records (EHR) systems, maintaining compliance with regulatory standards like HIPAA and state guidelines. Stay current with medical advancements through continuous education and contribute to quality improvement initiatives within the practice. Required Qualifications Active, unrestricted Nurse Practitioner or Physician Assistant license in Missouri. Master's degree or higher from an accredited NP or PA program. National certification as an NP (e.g., ANCC, AANP) or PA (NCCPA). Minimum of 1-2 years of clinical experience, preferably in primary care, geriatrics, or long-term care settings. Valid DEA registration and current BLS certification; ACLS preferred. Proficiency with EHR systems and telehealth platforms. Ability to travel within Windsor and surrounding areas as needed.
