Matrix Medical Network is seeking a dedicated Nurse Practitioner or Physician Assistant to join our clinical team on a PRN basis. In this essential role, you will travel to patients' homes throughout the Hayes Center, Nebraska region to perform comprehensive health assessments that support improved patient outcomes and care coordination. This flexible position allows you to leverage your clinical expertise while maintaining autonomy in your schedule.
As a PRN clinician, you will conduct thorough in-home evaluations that include obtaining detailed medical histories, performing physical examinations, identifying chronic conditions, and documenting findings using our electronic health record system. Your assessments will play a critical role in helping patients better understand their health status while providing valuable information to their primary care providers and healthcare teams. You will work independently, managing your own caseload while receiving support from our clinical and operational teams.
To succeed in this position, candidates must hold current, unrestricted licensure as a Nurse Practitioner or Physician Assistant in the state of Nebraska, along with national board certification in their respective discipline. A minimum of one year of clinical experience is preferred, though new graduates with strong assessment skills will be considered. Valid driver's license, reliable transportation, and the ability to travel within the designated service area are required. Proficiency with electronic health records and strong documentation skills are essential.
Successful candidates will demonstrate excellent clinical judgment, attention to detail, and the ability to work autonomously in diverse home environments. Strong interpersonal and communication skills are necessary to interact effectively with patients, families, and healthcare providers.
