Job Overview
Theoria Medical is seeking a dedicated Full-Time Nurse Practitioner (NP) or Physician Assistant (PA) to join our dynamic team in Eskridge, Kansas. As a key provider in our post-acute and long-term care network, you will deliver high-quality, comprehensive primary care services to patients in skilled nursing and assisted living facilities. Key Responsibilities
Conduct thorough patient assessments, including history taking, physical exams, and diagnostic evaluations to develop personalized treatment plans. Prescribe medications, order and interpret lab tests, imaging, and other diagnostics while adhering to evidence-based practices. Manage acute and chronic conditions such as diabetes, hypertension, COPD, and dementia-related issues, coordinating care with interdisciplinary teams including physicians, nurses, and facility staff. Perform procedures like wound care, joint injections, and minor surgeries as needed. Educate patients and families on health management, lifestyle modifications, and medication adherence to promote wellness and prevent hospital readmissions. Document all encounters accurately in electronic health records (EHR) systems, ensuring compliance with HIPAA, OSHA, and state regulations. Participate in on-call rotations and quality improvement initiatives to enhance patient outcomes and operational efficiency.
Required Qualifications
Active, unrestricted Nurse Practitioner or Physician Assistant license in Kansas; National certification (ANCC, AANP, NCCPA) required. Minimum of 2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings. Valid DEA registration and CPR/BLS certification. Proficiency in EHR systems and telehealth platforms. Must have reliable transportation and a clean driving record for travel between facilities in the Eskridge area.
