Job Overview Matrix Medical Network is looking for a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team on a PRN (as-needed) basis in Carrollton, IL. In this role, you will deliver comprehensive primary care services to patients in various community-based settings, including homes, senior living facilities, and clinics. Key Responsibilities Perform thorough patient assessments, including history taking, physical examinations, and diagnostic testing. Diagnose and treat acute and chronic conditions, prescribing medications and developing individualized care plans. Collaborate with interdisciplinary teams, including physicians, nurses, and specialists, to ensure coordinated care. Educate patients and families on health management, preventive care, and lifestyle modifications. Document all encounters accurately in electronic health records (EHR) systems, adhering to clinical guidelines and regulatory standards. Conduct health risk assessments and wellness checks, focusing on geriatric and adult populations. Respond promptly to urgent care needs and participate in quality improvement initiatives to enhance service delivery. Required Qualifications Active, unrestricted Nurse Practitioner or Physician Assistant license in Illinois. Minimum of 2 years of clinical experience in primary care, family practice, or internal medicine. Board certification (ANCC, AANP, NCCPA or equivalent). Valid DEA registration and prescriptive authority. Current BLS certification; ACLS preferred. Reliable transportation and ability to travel within the Carrollton area. Proficiency with EHR systems and telehealth platforms. Skills Strong clinical assessment and diagnostic skills.
