Job Overview
Salud Family Health, a leading Federally Qualified Health Center (FQHC) in Colorado, is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our Fort Morgan clinic. In this full-time position, you will play a vital role in delivering comprehensive primary care services to diverse patient populations, including underserved families, migrant workers, and low-income communities. Our mission is to provide accessible, high-quality healthcare with a focus on preventive care, chronic disease management, and holistic patient support in a collaborative, team-oriented environment.
Key Responsibilities
You will conduct thorough patient assessments, diagnose and treat acute and chronic illnesses, order and interpret diagnostic tests, prescribe medications, and develop individualized treatment plans. Responsibilities include performing physical exams, providing counseling on health maintenance and disease prevention, managing women's health services such as Pap smears and family planning, and participating in pediatric care. You will collaborate with multidisciplinary teams including physicians, nurses, behavioral health specialists, and support staff to ensure coordinated care. Additional duties involve maintaining accurate electronic health records (EHR), participating in quality improvement initiatives, conducting patient education sessions, and occasionally assisting with urgent care needs during clinic hours.
Required Qualifications
Candidates must hold a Master's degree or higher as a Nurse Practitioner or Physician Assistant from an accredited program, with current certification as an NP (via ANCC or AANP) or PA (via NCCPA). Active Colorado state licensure as an Advanced Practice Registered Nurse (APRN) or Physician Assistant, along with DEA registration and BLS certification, are required. A minimum of 2 years of clinical experience in primary care, family medicine, or community health settings is preferred, with experience serving diverse or underserved populations highly valued.
Skills
Strong clinical assessment and diagnostic skills, proficiency in EHR systems like Epic or similar, excellent communication and interpersonal abilities for building rapport with patients from varied backgrounds, cultural competency, and bilingual skills in Spanish are highly desirable. The ideal candidate demonstrates compassion, adaptability in a fast-paced clinic, commitment to evidence-based practice, and the ability to work independently while collaborating effectively in a team.
