Job Overview Theoria Medical is seeking a dedicated Part-Time Nurse Practitioner (NP) or Physician Assistant (PA) to provide exceptional primary care services in long-term care facilities in Sedan, KS. Manage acute and chronic illnesses common in geriatric populations, such as diabetes, hypertension, dementia, and respiratory conditions, with a focus on preventive health strategies. Educate patients, families, and staff on disease management, lifestyle modifications, and treatment adherence to promote better outcomes. Document all encounters accurately in electronic health records (EHR), ensuring compliance with regulatory standards and billing requirements. Participate in on-call rotations as needed and collaborate in quality improvement initiatives to optimize care delivery. Required Qualifications Active, unrestricted Nurse Practitioner or Physician Assistant license in Kansas, with national certification (ANCC, AANP, or NCCPA). Minimum of 1-2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings. Valid DEA registration and proficiency in EHR systems. Current BLS certification; ACLS preferred. Must be able to meet credentialing requirements and pass background checks.
