Full Description

Matrix Medical Network is a leading provider of in-home health assessments dedicated to improving patient outcomes through accessible, high-quality care. We partner with health plans and providers to deliver preventive services directly to patients where they live. As a PRN Nurse Practitioner or Physician Assistant, you will conduct thorough health assessments in patients' homes throughout the Hayes Center area. Your primary responsibilities include performing comprehensive physical examinations, reviewing medical histories, identifying potential health risks, and providing patient education on preventive care measures. You will document findings accurately in electronic health records and communicate results with the patient's primary care providers to ensure continuity of care. This role requires strong clinical assessment skills, attention to detail, and the ability to work independently while traveling to various home locations. You will manage your own schedule within PRN availability and ensure all assessments meet quality and compliance standards. Required qualifications include an active Nurse Practitioner or Physician Assistant license in Nebraska, current board certification, and at least one year of clinical experience. Candidates must possess a valid driver's license, reliable transportation, and the ability to pass background checks. Proficiency with electronic medical records and strong communication skills are essential. Key skills for success include clinical assessment expertise, patient education abilities, time management, documentation accuracy, and independent decision-making. Experience with geriatric or home health populations is preferred. Matrix Medical Network offers flexible PRN scheduling, competitive compensation, and opportunities for professional development. Team members benefit from ongoing training, support from clinical leadership, and the satisfaction of directly impacting patient health in their communities. Career growth opportunities include advancement into full-time clinical roles, leadership positions, or specialized care programs.
