Job Overview: The Partners Care is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our dynamic team in a full-time, fully remote position focused on Skilled Nursing Facilities (SNFs). This night shift role, from 7PM to 7AM, offers the flexibility of remote work while delivering critical healthcare services via telehealth to patients in SNF settings across various locations. Ideal for experienced clinicians passionate about geriatrics and post-acute care, you'll collaborate with interdisciplinary teams to manage complex cases, improve patient outcomes, and support facility staff remotely. Key Responsibilities: Conduct virtual assessments, diagnose acute and chronic conditions, and develop treatment plans for SNF residents during night hours. Prescribe medications, order diagnostic tests, and interpret results to guide patient care. Provide consultations to on-site nursing staff, respond to urgent calls, and ensure compliance with state and federal regulations. Document all encounters thoroughly in electronic health records (EHR), participate in care plan reviews, and coordinate with primary care providers or specialists. Maintain accurate records, adhere to evidence-based practices, and contribute to quality improvement initiatives aimed at reducing readmissions and enhancing resident quality of life. Required Qualifications: Active, unrestricted Nurse Practitioner or Physician Assistant license in good standing; at least 2 years of clinical experience, preferably in geriatrics, SNF, or post-acute care settings. Master's degree from an accredited NP or PA program; national certification (e.g., ANCC, AANP, NCCPA). Proficiency with telehealth platforms and EHR systems; current DEA registration and BLS certification required. Experience with Medicare/Medicaid documentation and familiarity with SNF workflows is highly preferred. Skills: Strong clinical judgment and decision-making in high-pressure, remote environments. Excellent communication and interpersonal skills for virtual interactions with patients, families, and staff. Technical proficiency in video conferencing, remote monitoring tools, and digital health records. Ability to work independently during night shifts while collaborating effectively in a team.
