Key Responsibilities: Conduct thorough patient assessments, diagnose acute and chronic conditions, and develop personalized treatment plans. Collaborate with interdisciplinary teams, including physicians, nurses, and facility staff, to coordinate care and ensure seamless transitions. Educate patients and families on health management, preventive care, and lifestyle modifications. Maintain accurate electronic health records (EHR) in compliance with HIPAA and state regulations. Participate in on-call rotations and quality improvement initiatives to enhance care delivery. Required Qualifications: Active, unrestricted Nurse Practitioner or Physician Assistant license in Kansas. Master's degree from an accredited NP or PA program. National certification (e.g., ANCC, AANP for NPs; NCCPA for PAs). Minimum of 1-2 years of clinical experience, preferably in primary care, geriatrics, or post-acute settings. Valid DEA registration and BLS certification; ACLS preferred. Strong knowledge of Medicare, Medicaid, and long-term care reimbursement guidelines. Skills: Exceptional clinical judgment and decision-making abilities. Proficiency in EHR systems such as Epic or Cerner.
