Job Overview
Ascension Health, a leading faith-based healthcare organization, is seeking a dedicated Registered Nurse Case Manager to join our team at our facility in Elk Grove Village, Illinois. In this full-time role, you will play a pivotal part in coordinating comprehensive patient care plans, optimizing resource utilization, and promoting positive health outcomes across diverse patient populations. As a key member of our interdisciplinary team, you will collaborate with physicians, social workers, and other healthcare professionals to ensure seamless transitions of care from hospital to home or community settings.
Key Responsibilities
You will conduct thorough patient assessments to identify clinical needs, risks, and barriers to care, developing individualized case management plans that align with evidence-based practices. Coordinate multidisciplinary care conferences, facilitate referrals to community resources, and monitor patient progress to adjust plans as necessary. Ensure compliance with regulatory standards, including payer requirements and quality metrics, while advocating for patients' rights and preferences. Educate patients and families on disease management, self-care strategies, and available support services to empower them in their health journey. Document all activities meticulously in electronic health records, contribute to discharge planning, and participate in quality improvement initiatives to enhance overall care delivery.
Required Qualifications
Candidates must hold a current Registered Nurse (RN) license in the state of Illinois, with a Bachelor of Science in Nursing (BSN) preferred. A minimum of 3 years of clinical nursing experience is required, ideally in case management, utilization review, or transitional care settings. Certification as a Case Manager (CCM) or similar credential is highly desirable. Proficiency in electronic medical records (EMR) systems and knowledge of healthcare reimbursement models, such as Medicare and Medicaid, are essential.
