Join Salud Family Health as a dedicated Nurse Practitioner or Physician Assistant providing essential primary care services to underserved communities in Fort Morgan, Colorado. This role offers the opportunity to deliver comprehensive healthcare to patients of all ages in a supportive clinical environment focused on accessible, quality medical care. Key Responsibilities include conducting thorough patient assessments through physical examinations and medical histories, diagnosing and treating acute and chronic health conditions, prescribing medications and developing individualized treatment plans, providing patient education on disease prevention and health maintenance, collaborating with physicians and healthcare team members to ensure coordinated care, maintaining accurate electronic health records and documentation, participating in quality improvement initiatives and continuing education activities, and supporting community health outreach programs as needed. Required Qualifications include current licensure as a Nurse Practitioner or Physician Assistant in the state of Colorado, national certification from an accredited certifying body, graduation from an accredited NP or PA program, and current DEA registration. Candidates must possess strong clinical assessment skills, excellent communication abilities, and a commitment to serving diverse patient populations with cultural sensitivity. Essential Skills include advanced clinical knowledge in primary care medicine, proficiency with electronic health record systems, strong diagnostic and treatment planning capabilities, effective patient education and counseling techniques, and the ability to work independently while collaborating within interdisciplinary healthcare teams. Bilingual Spanish-English skills are highly valued but not required.
