Job Overview Bloom Healthcare is looking for a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our primary care team in Saint Charles, Missouri. This full-time, home-based position offers the opportunity to deliver personalized, patient-centered care directly in patients' homes, making a meaningful impact on underserved and homebound individuals. As part of our innovative healthcare model, you'll collaborate with a multidisciplinary team to provide comprehensive primary care services, emphasizing holistic wellness, chronic condition management, and preventive health strategies. Key Responsibilities Conduct thorough patient assessments, including physical exams, medical histories, and diagnostic evaluations in home settings. Develop and implement individualized treatment plans, prescribe medications, order and interpret lab tests and imaging, and coordinate care with specialists and support services. Educate patients and families on disease prevention, self-management techniques, and lifestyle modifications to promote long-term health outcomes. Document all encounters accurately in electronic health records (EHR) systems, ensuring compliance with regulatory standards. Participate in on-call rotations as needed and collaborate with Bloom's care coordinators, physicians, and social workers to optimize patient care plans. Drive to multiple patient homes daily, managing a caseload efficiently while prioritizing safety and timely visits. Required Qualifications Active, unrestricted Nurse Practitioner or Physician Assistant license in Missouri; National certification (ANCC, AANP, NCCPA or equivalent). Minimum of 2 years of clinical experience in primary care, family medicine, or geriatrics; home-based care experience highly preferred. Valid driver's license, reliable vehicle, and auto insurance required for travel within Saint Charles and surrounding areas. DEA registration and proficiency in EHR systems such as Epic or Cerner. Skills Strong clinical assessment and diagnostic skills with expertise in adult and geriatric primary care. Excellent communication and interpersonal abilities to build rapport with diverse patient populations. Ability to work independently in dynamic home environments while maintaining high standards of care. Proficiency in telehealth integration and chronic disease management protocols. Compassionate, empathetic approach with solid problem-solving and critical thinking capabilities.
