Job Overview
Signify Health Medical Associates, PLLC is seeking an experienced Nurse Practitioner or Physician Assistant to join our clinical team on a per diem basis in Cambridge, Massachusetts. This role focuses on providing high-quality in-home health assessments, preventive screenings, and care coordination services to patients in their homes. The position offers the opportunity to work independently while delivering patient-centered care that improves health outcomes and supports better healthcare access.
Key Responsibilities
Conduct comprehensive in-home health assessments including medical history review, physical examinations, and vital signs monitoring. Perform preventive health screenings, identify chronic conditions, and develop care plans in collaboration with patients and their primary care providers. Document all clinical findings accurately in electronic health records and communicate assessment results to referring physicians. Educate patients on health management strategies, medication adherence, and lifestyle modifications. Coordinate with interdisciplinary care teams to ensure seamless transitions of care and appropriate follow-up services. Maintain compliance with all clinical protocols, quality standards, and regulatory requirements while delivering compassionate, patient-centered care.
Required Qualifications
Active and unrestricted Nurse Practitioner or Physician Assistant license in Massachusetts. Minimum of two years of clinical experience in primary care, internal medicine, geriatrics, or home health settings. Current BLS certification and valid driver's license with reliable transportation. Strong assessment, diagnostic, and clinical decision-making skills with proficiency in electronic health record documentation.
