Job Overview: Theoria Medical is seeking a dedicated Full-Time Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Eskridge, Kansas. In this role, you will provide comprehensive primary and urgent care services to patients in long-term care facilities, ensuring high-quality, patient-centered healthcare in a collaborative environment. Key Responsibilities: Conduct thorough patient assessments, including history taking, physical examinations, and diagnostic evaluations. Develop and implement individualized treatment plans, prescribe medications, and order appropriate diagnostic tests. Manage acute and chronic illnesses, perform minor procedures, and coordinate care with specialists and facility staff. Educate patients and families on health maintenance, disease prevention, and treatment adherence. Maintain accurate electronic health records, ensure compliance with regulatory standards, and participate in quality improvement initiatives. Collaborate with interdisciplinary teams to optimize patient outcomes and provide on-call coverage as needed. Required Qualifications: Active, unrestricted Nurse Practitioner or Physician Assistant license in Kansas. Master's degree or higher from an accredited NP or PA program. Minimum of 1-2 years of clinical experience, preferably in primary care, geriatrics, or long-term care settings. Current DEA registration and BLS certification; ACLS preferred. Valid NPI number and ability to pass background checks. Skills: Strong clinical assessment and diagnostic skills. Excellent communication and interpersonal abilities for patient and team interactions. Proficiency in electronic medical records (EMR) systems and telehealth platforms.
