Job Overview Matrix Medical Network is looking for a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our dynamic team in Luray, Virginia. In this role, you will provide comprehensive primary and urgent care services to patients in home-based and community settings, focusing on preventive health, chronic disease management, and health promotion. As a key member of our interdisciplinary team, you will conduct thorough assessments, diagnose conditions, prescribe treatments, and collaborate with healthcare professionals to improve patient outcomes in underserved populations. Key Responsibilities Your primary duties will include performing detailed patient evaluations, including history taking, physical examinations, and diagnostic testing interpretation. You will develop and implement individualized care plans, prescribe medications, order lab tests and imaging, and provide patient education on disease prevention and self-management. Additionally, you will document all encounters accurately in electronic health records, coordinate care with primary care providers and specialists, participate in quality improvement initiatives, and maintain compliance with all regulatory standards. Expect to travel within the Luray area to patient homes or senior living facilities, adapting to diverse patient needs with cultural sensitivity and empathy. Required Qualifications Candidates must hold a valid Nurse Practitioner or Physician Assistant license in Virginia, with national certification from ANCC, AANP, NCCPA, or equivalent. A Master's or Doctorate degree in Nursing or Physician Assistant studies from an accredited program is required, along with at least 2 years of clinical experience in primary care, family practice, or geriatrics. Current DEA registration and BLS certification are mandatory, with experience in home health or mobile care preferred. Skills Success in this role demands strong clinical assessment and diagnostic skills, excellent communication and interpersonal abilities to build rapport with patients and families, and proficiency in electronic medical records systems like Epic or similar. You should demonstrate decision-making prowess under minimal supervision, cultural competence, time management for a mobile schedule, and a commitment to evidence-based practice. Familiarity with telehealth platforms and chronic condition management is highly advantageous.
