Job Overview Salud Family Health, a trusted provider of comprehensive healthcare services in underserved communities, is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our Fort Morgan, Colorado clinic. In this full-time role, you will deliver primary care to diverse patient populations, emphasizing preventive health, chronic disease management, and holistic wellness in a supportive, patient-centered environment. Key Responsibilities Conduct thorough patient assessments, including history taking, physical examinations, and diagnostic testing interpretation. Diagnose and treat acute and chronic illnesses, prescribe medications, and develop individualized treatment plans. Perform minor procedures, such as suturing, biopsies, and joint injections, while educating patients on health maintenance and disease prevention. Collaborate with multidisciplinary teams, including physicians, nurses, and specialists, to coordinate care and refer patients as needed. Maintain accurate electronic health records (EHR), ensure compliance with HIPAA and clinic protocols, and participate in quality improvement initiatives. Engage in community outreach to promote health equity and provide culturally sensitive care to our Spanish-speaking and immigrant populations. Required Qualifications Master's or Doctorate degree in Nursing (for NPs) or Physician Assistant Studies from an accredited program. Current, unrestricted Colorado NP or PA licensure, DEA registration, and national certification (ANCC, AANP, NCCPA). Minimum of 2 years of clinical experience in primary care, family medicine, or internal medicine preferred. BLS certification required; ACLS preferred. Skills Excellent clinical judgment and decision-making abilities. Strong communication and interpersonal skills for building patient rapport. Proficiency in EHR systems like Epic or Cerner. Bilingual (English/Spanish) highly desirable. Ability to thrive in a fast-paced, high-volume clinic while prioritizing empathy and cultural competence.
